Forward your incoming calls and SMS messages with Advanced Forwarder App for BlackBerry
I liked this App which gives you full control for forwarding not only your incoming calls but also your text messages which can be instantly enabled or disabled both locally from the application interface or remotely with an SMS command sent from another phone.

Downside
- Does not support all CDMA carriers

I am not sure but it looks like this app will not work for conditional forwarding. (As per my knowledge, this conditional forwarding any way is not fully supported by most mobile carriers for BlackBerry devices)
Be sure to check our if your carrier/ device is supported by this application and keep in mind that full version is good for that particular BlackBerry only. If you change the BlackBerry then you will have to buy this application again.
This BlackBerry App will be very helpful for those who are always on the go and have multiple contact numbers and enable/ disable call forwarding multiple times. This app provides free trial for 30 calls/ messages.
